How do I make an appointment?
Please call (605) 322-6895 to work with an Avera concierge, who will help you select a time that best fits your schedule.
When your appointment has been scheduled, you will receive a personal health history log. On this form you can provide medical information that will help in creating an individualized physical assessment for you.
When we receive your completed information, you will also be given complete instructions on how to prepare and what you can expect on the day that you are our guest at Avera Executive Health.
