Our Purpose
Our goals at Avera McKennan Home Care are to:
- Provide continuity of care from hospital to home
- Shorten hospital stays or prevent hospitalization
- Expedite recovery and return to regular activities of daily living
- Reduce the cost of illness
- Collaborate with other community agencies to facilitate providing skilled care in the home
Home Care services are available 24 hours a day. Regular office hours are from 8 AM to 5 PM, Monday through Friday. We are located at 1016 S. Cliff Avenue in Sioux Falls.
