Home Care Referral Information

Referral to Avera McKennan Home Care can be made by physicians, patients, patient’s family, friends or community agencies.

• The patient must be under the care of a physician who will write an order for home care staff. A patient must live in the program service area, which consists of the Sioux Falls community and surrounding 40-mile area extending into Minnesota and Iowa.
• Patients living outside of the Avera McKennan Home Care service area are referred to local home care agencies located near the patient’s home.
• All patients referred to Home Care will have a pre-admission interview by a RN from Home Care to determine if Home Care is appropriate.
