Privacy

Our patients' privacy is important to us. On your first appointment to Family Medicine Clinics, you will be asked for basic information to establish your patient account including an Acknowledgement of Privacy Notice and Consent to Treat Form .  You may print these off, fill out and bring with you to your first appointment.  Our privacy policies are outlines in our Notice of Privacy Practices brochure which is also available in our waiting room.

Doctor/Patient Relationship

The relationship between our physicians and their patients is confidential. It is a relationship based on mutual trust and understanding.

Your medical records are absolutely private. No information about your condition will be given to any third party without your permission (except if required by a court of law).

Workers' compensation claims are the exception. These types of claims are subject to review by employers and/or their insurance carriers.
