OPTIFAST Program Entry
Please read through the following questions to determine your program eligibility.
- Are you 18 years or older?
- Is your BMI > 30 or do you have over 50 pounds of weight to lose? (To determine your BMI please use the link to the right).
- If you answered "no" to the two previous questions, please contact the ACMN staff at (605) 322-3091 to determine other weight loss options. If you answered "yes" to the two previous questions, please continue to the next set of questions.
- Do you have a history of strokes?
- Do you have diabetes mellitus?
- Do you use steroid medications (> 20 mg/day)?
- Have you had a bleeding ulcer in the last 6 months?
- Do you have an active blood clot?
- Are you pregnant, breast feeding or considering becoming pregnant?
If you answered "yes" to any of the previous questions, please contact ACMN at (605) 322-3091 to determine if you qualify for the program.
If you answered "no" to each of the previous questions, please continue to the next set of questions.
- Do you have a history of mini-strokes?
- Do you have a history of blood clots?
- Do you have any occupational hazards (any job that requires a person to be alert at all times, ex. surgeon, pilots, bus drivers, truck drivers, window washers)?
- Are you using drugs with any potent gastro-intestinal side effects?
- Do you have a history of liver or kidney disease?
If you answered "yes" to any of the previous questions please contact ACMN staff to determine your program eligibility.
If you answered "no" to each of these questions, please continue to the next set of questions.
Psychiatric History:
- Do you have a psychiatric history that includes suicide attempts?
- Are you currently taking lithium carbonate?
- Are you currently taking psychotropic medications (to treat depression, bipolar disorder, schizophrenia, or other psychiatric diagnosis)?
